Definition: Estimated percentage of children with special health care needs ages 12-17 who receive services necessary to make appropriate transitions to adult health care, work, and independence, by type of health insurance coverage (e.g., in 2009-2010, 23.8% of publicly-insured youth with special health care needs in California received transition to adulthood services).
