Definition: Estimated percentage of children ages 0-17 with special health care needs (CSHCN) who receive and do not receive coordinated, ongoing, comprehensive health care within a medical home, by family income level (e.g., in 2016-2020, among California CSHCN with family incomes at or above 400% of their federal poverty threshold, 52.4% received care within a medical home).

Footnote: Due to changes in methodology, these estimates should not be compared with data from earlier years. Children with special health care needs (CSHCN) have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and require health and related services of a type or amount beyond that required by children generally. The federal poverty threshold was $26,246 for a family of two adults and two children in 2020. A medical home is a model of delivering primary care that is accessible, family centered, continuous, comprehensive, coordinated, compassionate, and culturally effective. In these estimates, children receiving care within a medical home must meet criteria for three components of primary care: (i) personal doctor or nurse, (ii) usual source for sick care, and (iii) family-centered care. Children needing referrals to specialty care and/or care coordination must also meet criteria for those components. These estimates are based on a survey of the population and are subject to both sampling and nonsampling error. The notation S refers to estimates that have been suppressed because the margin of error is greater than 10 percentage points.
