Definition: Estimated percentage of children ages 0-17 with special health care needs (CSHCN) who receive coordinated, ongoing, comprehensive health care that meets American Academy of Pediatrics 'medical home' standards, by household income level (e.g., in 2022, among California CSHCN living on income at or above 400% of their household's federal poverty threshold, 39.9% received care within a medical home).

Footnote: A medical home is a model of delivering primary care that is accessible, family centered, continuous, comprehensive, coordinated, compassionate, and culturally effective. In these estimates, children receiving care within a medical home must meet criteria for three components of primary care: (i) personal doctor or nurse, (ii) usual source for sick care, and (iii) family-centered care. Children needing (iv) referrals to specialty care and/or (v) care coordination must also meet criteria for those components. Children with special health care needs (CSHCN) have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and require health and related services of a type or amount beyond that required by children generally. The federal poverty threshold was $29,678 for a family of two adults and two children in 2022. These estimates are based on a survey of the population and are subject to both sampling and nonsampling error. The notation S refers to estimates that have been suppressed because (a) there were fewer than 20 respondents in that group, or (b) the margin of error for the estimate is greater than 10 percentage points. The annotation [!] indicates that the estimate's margin of error is greater than 5 percentage points but not greater than 10 percentage points.
